The Management of Peripheral Arterial Emboli.

چکیده
ONE of the main difficulties in assessing the results of any surgical procedure is the provision of a directly comparable series of patients on whom it has not been employed. Nowhere is this more evident than in trying to determine the value of embolectomy in the management of peripheral arterial emboli. The problem of comparing the results of conservative and operative management is considerable, due to the fact that conservative treatment is employed in poor risk patients in whom surgery of any kind is inadvisable and also in those patients whose ilimbs are recovering spontaneously when they are first seen. Peripheral arterial embolism is a grave condition. This is evidenced by the fact that in most published series of cases the mortality rate is in excess of 40 per cent whether embolectomy has been employed or not. It has even been suggested that in some series where surgical intervention was extensively employed, the mortality rate was higher than if conservative management alone was used. However, we feel this does not necessarily mean that surgery should not be advised. Although the overalil results are poor in terms of mortality and loss of limb attributable to the natural history of the disease, a worthwhile result can be achieved in a minority of patients whose limb would not otherwise be restored to normal function. Tihe problem of selection of those patients who are likely to benefit from embolectomy is difficult if not impossible. It is therefore inevitable that the procedure will be employed in a number of instances where the eventual outcome is fatal.

Microemboli in our bypass circuits: a contemporary audit.
Cardiopulmonary bypass (CPB) may introduce microemboli into the patient's arterial circulation. These may arise from the CPB circuit. Most relevant studies have been performed in vitro; there are relatively few clinical studies. We used the Emboli Detection and Classification quantifier (EDAC) (Luna Innovations, Roanoke, VA) in a prospective clinical audit of emboli in a contemporary CPB circui...
متن کاملAnalysis of Risk Factors for Amputation in 822 Cases with Acute Arterial Emboli
BACKGROUND We retrospectively examined the records of 822 patients who underwent a total of 901 operations for acute peripheral arterial occlusion of the upper or lower extremities between 1999 and 2009. We analyzed the effects of atherosclerotic structure, the time of admission to hospital, and re-embolectomies on amputation in the early postoperative period. METHODS There were 466 (56.7%) m...
متن کاملBlood temperature management and gaseous microemboli creation: an in-vitro analysis.
Gaseous microemboli have been associated with post operative neurological deficits in patients undergoing cardiopulmonary bypass. Creating an optimal perfusion system that minimizes microemboli production and has enhanced abilities to sequester entrained air during the bypass procedure has been an important focus. This study examines the air-handling capabilities of a cardiopulmonary bypass cir...
متن کاملEmbolic Protection in Femoropopliteal Artery Intervention
F emoropopliteal occlusive disease is a major contributor to claudication and critical limb ischemia and is present in 20% to 40% of patients with peripheral arterial disease (PAD). As the population ages and technology advances, the number of endovascular treatments performed for femoropopliteal lesions is expected to increase. Endovascular recanalization and treatment of the superficial femor...
متن کاملDiagnosis, Prognosis and Treatment of Peripheral Vascular Disease in Patients with Diabetic Foot Ulcer: Adopted IWGDF Guidance
Introduction: Peripheral arterial disease (PAD) is common in diabetic patients, and about half of the patients with diabetic foot ulcers (DFU) have concomitant peripheral arterial disease (PAD). The purpose of this study was to select an appropriate and comprehensive clinical guide in the management of DFU. The part of the practical guide that pertains to this is based on three systematic revie...
